  4. Describe the youth triathlete’s involvement with an established swimming or triathlon program, and include the names and contact information for the coaches/instructors/ organizers and their credentials (e.g., USAT Certified Coach). Be sure to describe frequency of training sessions and the duration of the program (e.g., year-round). Please include frequency of competitions.

  6. Present official race results for the youth triathlete from recent triathlons similar in distance to the race you wish to register for. Please include the name of the race, date, distance for each discipline and your time. You may list the URL for the results page from the website of the race (e.g., http://longleaftriathlon.com/results).

Provide a written endorsement from a Coach certified by USA Swimming or USAT, stating his/her assessment of the youth triathlete’s ability to easily complete the swim portion of the triathlon within the variable open water conditions of an adult triathlon. Please be sure the name, credentials and contact information for the Coach are included.

  10. Please provide official race results documenting the youth triathlete has competed in an event of 400-yards or more, in a sanctioned USA Swim meet, within the last three months.

  19. Deadline for youth athlete applications is September 30, 2011. All youth athletes must present a photo ID and USAT membership card at packet pick up or purchase either a one-day or annual membership that day.
